For and in consideration of me and/or my child or children listed below being permitted to enter into an area of the ALERT Airfield in Upshur County, Texas from which the general public is normally excluded or restricted, and participate in aviation activities the week of October 7-13, 2023 (the “Event”), which activities include: Observing airplanes up close and in operation, and/or riding in one or more airplanes while taxiing and in flight, I hereby GENERALLY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY, DEFEND AND SAVE HARMLESS, all participants in the Event, the sanctioning organization, the owners and the pilots of all aircraft participating, The Institute in Basic Life Principles, International ALERT Academy, Christian Family Fellowship, its board members, employees, volunteers, owners, or any person or persons connected with the said ministries (the “Releasees”), from all liability to the undersigned and his or her personal representatives, assigns, heirs and next of kin, and all other persons, for any and all loss or damage and any suits, claims or demands on account of any injury to or death of the undersigned or any persons of the undersigned, whether caused by the negligence of the Releasees or otherwise, I realize that the Event activities involve risks and dangers of personal injury and death, and that there may be other risks and dangers not known to me or not reasonably foreseeable at this time. I understand this event has no cost to me other than the fact I will be paying equal shares of operating expenses for any aircraft ride that I may choose to participate under part 91 of the FARs. (Estimate cost: $30/person)

As the parent of my child or children listed below, I certify that I am his, her or their parent and/or legal guardian, and that I have the right to give, and do hereby give, permission for my child or children to participate in the Event activities, and I agree that, in the event any claim for personal injury, property damage, or wrongful death is prosecuted against any of the Releasees by, for, or on behalf of, my child or children, or third parties on account of any injury or death, I will indemnify and hold the Releasees
harmless.
